Sign Up for Health Insurance

Come sign up for Health Insurance with Covered California, a statewide insurance exchange under the new Affordable Care Act.  Certified enrollment counselors will be available to help you learn which option* is the best fit for your family and help you complete the necessary online or paper application.

Two sessions!

When: Saturday, December 14, 8am-3pm
Where: Palo Alto Unified School District Office, 25 Churchill Avenue

When: Thursday, December 19, 2pm-7pm
Where: Jordan Middle School Multi-Purpose Room, 750 N. California Avenue

Spanish language translation, childcare, and plenty of enrollment counselors available.
REQUIRED documents needed for enrollment include:
-Current income of all family members on the application (Tax Return, W2, recent pay stubs, etc.)
-Copy of US Citizenship and residency status
-Legal resident card or Certificate of Naturalized Citizenship
-Copy of social security number and date of birth for each family member in the household
-Applicant's California ID or Driver's License

*The Affordable Care Act provides two paths to help uninsured people get health insurance:  You may buy a plan from Covered California or you may be covered by an expansion of Medi-Cal, the government's health insurance program.  Your income will determine which of these paths is right for you.
